7-10 days. May consider 14, if patient severely ill
If unable to take PO, may consider Ceftriaxone 2g q24h and de-escalate based on culture results
Nitrofurantoin 100mg BID x 7 days
Do NOT use for CrCl < 30 mL/min
OR
SMX/TMP 1 DS tab BID x 7 days
Cephalexin 1000mg TID x7 days
Amoxicillin-clavulanate 875/125mg BID x7 days
Cefuroxime 250mg BID x7 days
Cefdinir 300mg BID x7 days
Ciprofloxacin 250mg BID x7 days
Use only if severe allergy to other agents, Pseudomonas aeruginosa history or non-removable device (e.g., ureteral stent)
Levofloxacin 250mg daily x7 days
