Outpatient - Bacterial Pneumonia

Duration of Treatment

7-10 days

Preferred Regimen

Amoxicillin 90 mg/kg/day PO in 2 divided doses (up to 2000 mg/dose)

CONSIDER ADDING*

Azithromycin 10 mg/kg on day 1, then 5 mg/kg/day on days 2-5

*For children 5 years old and older, if unable to determine bacterial pneumonia versus atypical pneumonia

Alternative Regimen

Amoxicillin-clavulanate 90 mg/kg/day PO in 2 divided doses up to 4000 mg/day (use 600 mg/5mL susp)

OR

Cefuroxime 30 mg/kg/day in 2 divided doses (max 500 mg/dose)

OR

Cefdinir 14-28 mg/kg/day in 2 divided doses (max 300 mg/dose)

For Suspected CA-MRSA

ADD

Vancomycin 60 mg/kg/day IV in 4 divided doses

OR

Trimethoprim-sulfamethoxazole 8-10 mg/kg/day TMP component in 2 divided doses (max 320 mg TMP/dose)
