CefoTAXime
For less than 90 days corrected age
50 mg/kg/dose IV q8h or 37.5 mg/kg/dose IV q6h Max: 12 g/day, 4g/dose
OR
CefTRIaxone
For greater than/equal to 90 days corrected age
50-75mg/kg/dose IV q24h Max: 1 g/day
Gentamicin/Tobramycin
If weight MORE than 5 kg 5-7.5 mg/kg IV q24h Max: 800 mg/dose prior to drug level monitoring
If weight LESS or equal to 5 kg 2.5 mg/kg/dose IV q8h Max: 120 mg/dose prior to drug level monitoring
Drug Level Monitoring
ADD TO ANY OF THE ABOVE*
Ampicillin 50 mg/kg/dose IV q6h Max: 1 g/dose, 4 g/day
*If prior urine colonization with enterococcus or septic patients with suspected UTI
