Outpatient

Empiric Regimen

SMX/TMP 1 DS tab BID x14 days

OR

Cefdinir 300mg BID x14 days

OR

Cephalexin 1000mg TID x14 days

OR

Ciprofloxacin 500mg BID x7 days

Use only if severe allergy to other agents or non-removable device (e.g. ureteral stent)

OR

Levofloxacin 750mg daily X5 days

Use only if severe allergy to other agents or non-removable device (e.g. ureteral stent)

Optional IV Dose

Ceftriaxone 2g IV x1 day

OR

Ciprofloxacin 400mg IV x1 day

Use only if severe allergy to other agents or non-removable device (e.g. ureteral stent)
