Comorbidities

  • Chronic heart, lung, liver, or renal disease

  • Diabetes mellitus

  • Alcoholism

  • Malignancy

  • Asplenia

Empiric Therapy With No Comorbidities

Azithromycin 500mg IV/PO initial dose then 250mg daily x 5 days

OR

Doxycycline 100 mg PO every 12 hr

Empiric Therapy With Comorbidities

Augmentin 875 mg/125 mg PO every 12 hr

PLUS

Azithromycin 500mg IV/PO initial dose then 250mg daily x 5 days

OR

Cefuroxime 500 mg PO every 12 hr

PLUS

Azithromycin 500mg IV/PO initial dose then 250mg daily x 5 days

Macrolide Allergy

In combination therapy, if a macrolide allergy exists, doxycycline can be substituted for azithromycin

Doxycycline 100 mg PO every 12 hr

Duration

5 days total therapy
