Preferred Regimen

Ampicillin 200 mg/kg/day IV in 4 divided doses (up to 2000 mg/dose)

Alternative Regimen

Ceftriaxone 50 mg/kg daily IV/IM (max 2000 mg)

For Suspected CA-MRSA

ADD

Vancomycin 60 mg/kg/day IV in 4 divided doses

OR

Trimethoprim-sulfamethoxazole 8-10 mg/kg/day TMP component in 2 divided doses (max 320 mg TMP/dose)
