Patients with severe infection are at risk of abdominal compartment syndrome - monitor intrabdominal pressures if possible
Abdominal Compartment Syndrome
Abdominal compartment syndrome (ACS) is defined as intra-abdominal pressure >20mmHg and new organ failure (renal, pulmonary or hemodynamic).
If ACS is suspected, consult surgery
Duration of therapy depends on clinical course
In patients who fail to improve, surgical debridement of necrosis may be required and consultation with surgical team recommended
Meropenem 1g IV q8h or 500 mg IV q6h
If patient not responding to antibiotics and has Candida risk factors consider empiric Caspofungin 50mg IV q24h
Candida risk factors
Candida colonization of multiple body sites
Immunocompromise
Broad-spectrum antibiotic use
Abdominal surgery
Central venous catheters
TPN
