Treatment involves eradication of GAS carriage and management of hypertension
Eradication of GAS carriage does not alter the natural history of the disease, but prevents the spread of nephritogenic strains of group A Streptococcus
Phenoxymethyl Penicillin
1-5 years: 125mg, 4 times/day for 10 days
6-12 years: 250mg, 4 times/day for 10 days
>12 years: 500mg, 4 times/day for 10 days
OR
Erythromycin 30-50 mg/kg/day PO in 4 divided doses
If allergic to penicillin
Significant but asymptomatic hypertension
Hypertensive emergency
Characterized by raised blood pressure with end organ damage such as cerebral infarction, cerebral hemorrhage, encephalopathy (seizures), pulmonary oedema, and also symptomatic
