In patients with severe systemic illness, therapy should be initiated with a parenteral agent

Imipenem or Aztreonam are potential alternative second line drugs; however experience of their use is limited and their efficacy is not guaranteed in ceftriaxone-resistant infections
Corticosteroid therapy may decrease mortality in severe enteric fever and should be considered in children with altered mental state
The recommended dose in adult patients is dexamethasone 3 mg/kg stat followed by another 8 doses of 1 mg/kg 6Hrly
There is insufficient evidence to recommend a different dose of dexamethasone in paediatric patients
Request a surgical opinion in any patient with suspected intestinal perforation or lower gastrointestinal bleeding
