Empiric Regimen

Meropenem 1g iv q8h

Please use Antimicrobial Restriction Order Set

OR

Pip/tazo 4.5g iv q6h

If Legionella Suspected, Add

Levofloxacin 750mg iv q24h

If MRSA Suspected, Add

Vancomycin IV

Please use Vancomycin Order Set

If Pseudomonas Suspected, ADD

Pip/tazo 4.5g iv q6h

If not already using

OR

Tobramycin 5-7mg/kg iv q24h

OR

Gentamicin 5-7mg/kg iv q24h

OR

Levofloxacin 750mg iv q24h

If not already using

OR

Ciprofloxacin 400mg iv q8h
