Female Uncomp Cystitis Rx

Empiric Rx

Review previous urine culture data if recurrent UTI (patient specific antibiogram) to better inform empiric choice

Nitrofurantoin 100mg PO BID x 5-7 days unless early pyelo or eGFR <60

OR

If minimal previous antibiotic exposure: TMP-SMX 1 DS PO BID x 3d

OR

Fosfomycin 3g PO once, especially if risk of resistant organism, including recurrent UTI, antimicrobial exposure, and travel

OR

Alternative: Levofloxacin 250 mg PO daily x 3 days, but should reserve fluoroquinolones for other uses
