Serious Type II-IV Reaction

  • Serum sickness

  • Stevens-Johnson syndrome

  • Toxic Epidermal Necrolysis

  • Drug Rash Eosinophiia Systemic Symptoms (DRESS)

Type I Reaction

  • Anaphylaxis

  • Angioedema

  • Wheezing

  • Laryngeal edema

  • Hypotension

  • Hives/Uticarial

OR

  • Unknown reaction WITHOUT muscosal involvement, skin desquamation or organ involvement

Mild Reaction

  • Minor rash

  • GI symptoms only

  • Family history of allergy

  • Record lists allergy, but patient or caregiver denies
