Criteria

  • Failure of oral antibiotics, or

  • Systemic signs of infection

    • Temp >38°C
    • Tachycardia
    • Tachypnea
    • Abnormal white blood cell count

Initial Empiric Treatment

Cefazolin IV

Alternative

Clindamycin IV

If allergy to Cephalexin/Amoxicillin

PO Switch

May switch to PO if tolerating PO, afebrile, and signs of skin infection improving

Once susceptibilities are available, de-escalate to narrowest spectrum PO antibiotic
