Post-op IAI: Severe infection

Duration

4-7 days

  • Patients with adequate source control procedure (e.g., surgery, IR drainage, ERCP) can have their antibiotics discontinued 4 days after the procedure.

  • Patients who DO NOT have adequate source control may require prolonged course of antibiotics dependent on clinical and radiographic response. Consider expert
    consultation.

Empiric Regimen

Meropenem 500 mg IV q6h

AND

Vancomycin

If yeast or candida identified ADD:

Micafungin 100 mg IV q24h
