Empiric Treatment

Ceftriaxone 2 g IV q24h

AND

Metronidazole 500 mg PO/IV q12h

OR

Piperacillin-tazobactam 3.375 g IV q6h

AND

If MRSA suspected:

Vancomycin

MRSA coverage can be discontinued after 48 hours if cultures don’t show MRSA

If ESBL colonized or infected

Meropenem 500 mg IV q6h

Severe beta-lactam allergy

Moxifloxacin 400 mg PO/IV q24h
