Common Presentation

Fever, abdominal cramps, and diarrhea within 6-48 hours, fecal leukocytes often present

Commonly Implicated Sources and Seasonality

Poultry, eggs, dairy products, reptile contact

Peak season – spring, summer

Treatment Recommendations

Antibiotics have no significant effect on the length of illness and may prolong carriage of the organism in the stool

Antibiotics should generally be avoided but recommended for severe illness (>8 stools/day, high fever, hospitalized) or risk for complications (age < 3 months, immunocompromised) or disseminated disease.
In this instance ceftriaxone is the drug of choice.

CBC plus blood culture should be considered for infants or immunocompromised patients who continue to have fever by time of diagnosis

Duration of Therapy

Immunocompromised patients require 14 days of therapy or longer if relapsing

Antibiotics (if Indicated) for step down therapy
