Suggested regimen

For patients less 35 kg
Dosing based on amoxicillin component:
Amoxicillin-Clavulanate 16 mg/kg/dose PO TID
Use the 80 mg/mL (7:1) formulation only

OR

For patients greater than 35 kg:
Amoxicillin-Clavulanate 500 mg PO TID
Use 500 mg tablets (4:1) or the 80 mg/mL (7:1) formulation only

Penicillin allergy

Clindamycin 20-30 mg/kg/24h PO divided TID-QID
Maximum: 1.8 grams/24h

AND

Dosing based on trimethoprim component
Co-trimoxazole 4-6 mg/kg/dose PO BID
