Most likely pathogens
Febrile: 7-10 days Febrile urinary tract infection indicates there is a suspected or confirmed pyelonephritis.
Afebrile not systemically ill, not recurrent, normal urinary tract anatomy, no history of resistant pathogens, normal renal function : 5-7 days

Cephalexin 12.5 mg/kg/dose PO QID Maximum: 500 mg/dose
OR
Cefixime 8 mg/kg/dose PO daily Maximum: 400 mg/dose
Amoxicillin 16 mg/kg/dose PO TID Maximum: 500 mg/dose (Do not use empirically as there are high rates of resistance with E. Coli and poor activity against Klebsiella)
Dosing is based on trimethoprim component: Co-trimoxazole 4 mg/kg/dose PO BID
