Urinary Tract Infections

Note

Cystitis or step down therapy from pyelonephritis.

Treatment should be guided by susceptibility testing

Bacterial Pathogen

Escherichia coli

Enterococcus sp.

Klebsiella sp.

Proteus sp.

Pseudomonas sp.

Duration of Therapy

7-10 days

First Line Therapy

Cotrimoxazole 6-12 mg TMP/kg per day divided bid

OR

Amoxicillin/Clavulanic acid: 40-50 mg/kg per day divided tid

OR

Cefixime 8 mg/kg per day q12-24h

Alternatives (in Order)

Amoxicillin 40-50 mg/kg per day divided tid

OR

Cephalexin 25-100 mg/kg per day divided qid

OR

Nitrofurantoin 5-7mg/kg/day PO divided qid*

*Infants older than 1 month

*To be used in infections of the lower urinary tract only.
