Renally cleared, requires dosage adjustment with changes in renal function. Consult a pharmacist for renal dosing.
PO 20-30 mg/kg/DAY divided q12h
Low dose: PO 40-50 mg/kg/DAY divided q8h High dose: PO 80-90 mg/kg/DAY divided q8h. High dose can be divided q12h for otitis media. Max 4 g/DAY.
Therapy of Streptococcal, Enterococcal infections as well as susceptible gram negative infections.
Commonly used for upper respiratory infections and Urinary tract infection
Allergy/rash (immediate or delayed), cytopenias.
EBV related rash (not allergy).
Antimicrobial class: Aminopenicillin
Average serum half life: Neonates, full-term: 3.7 hours. Infants and children: 1 to 2 hours
Route of Elimination: Renal excretion (60% unchanged drug)
