Cefazolin IV 30 mg/kg

Clindamycin IV 10 mg/kg

Pre-Op Administration

Administer over 30 minutes just prior to procedure

ALSO CONSIDER

Gentamicin IV 2.5mg/kg*

*Gentamicin dosing should be based on actual body weight (ABW), or dosing weight (DW) if patient's ABW is > 25% above IBW (Ideal Body Weight). Consult a pharmacist regarding a patient with renal insufficiency.
