Spinal Surgery

Note

MRSA carrier: Consider intranasal mupirocin 2% bid-tid X 3-5 days prior to surgery.

Suggested Regimen

Cefazolin IV 30 mg/kg

Pre-Op Administration

Direct IV

Intraoperative Re-Dosing Interval

q4h from time of pre-op dose

Post-Op

Cefazolin q8h to maximum of 24h post-op

Penicillin Allergy/Anaphylaxis

Vancomycin IV 15mg/kg*

*Vancomycin dosing should be based on actual body weight. Consult a pharmacist regarding a patient with renal insufficiency.

Pre-Op Administration

Administer over 60 – 90

minutes, beginning 1-2 hours before incision.

Intraoperative Re-Dosing Interval

q6h from time of pre-op dose

Post-Op

Vancomycin q8h to a maximum of 24h post-op
