Cefazolin IV 30 mg/kg

Vancomycin IV 15mg/kg*
*Vancomycin dosing should be based on actual body weight. Consult a pharmacist regarding a patient with renal insufficiency.
Administer over 60 – 90
minutes, beginning 1-2 hours before incision.
