  • central venous catheter

  • placement carotid endarterectomy

  • brachial artery repair

  • endovascular stenting

Suggested Regimen

Cefazolin IV 30 mg/kg

Pre-Op Administration

Direct IV

Intraoperative Re-Dosing Interval

q4h from time of pre-op dose

Penicillin Allergy/Anaphylaxis

Clindamycin IV 10 mg/kg

Pre-Op Administration

Administer over 30 minutes just prior to procedure

q4h from time of pre-op dose
