APTP or Admit
Do blood culture
Ceftriaxone 50 mg/kg IV once daily
OR
Gentamicin 5 to 7 mg/kg IV every 24 hours
Gentamicin Serum Level Monitoring
Consider obtaining gentamicin trough level (prior to any dose) and monitoring serum creatinine in the following circumstances. Target trough level is < 1 mcg/mL (or 1mg/L):
Patient at high risk of nephrotoxicity or renal dysfunction (e.g. patient on other nephrotoxic medications)
Duration of treatment with gentamicin anticipated to be > 5 days
