Dose to a target trough level of 15-20mg/L

PLUS EITHER

Cloxacillin 2g IV Q4H

OR

Cefazolin 2g IV Q8H

MRSA

Continue Vancomycin

MSSA

Cloxacillin 2g IV Q4H

OR

Cefazolin 2g IV Q8H

Duration of Therapy

Minimum 14 days from negative blood culture
