Notes

Dosages recommended are for patients with normal renal function

Fluoroquinolones are only recommended for patients unable to tolerate cephalosporin and ampicillin therapy.

FQs are generally not recommended for patients under 18 years old.

Duration

Native valve: 4-6 weeks

Prosthetic valve: 6 weeks

Antibiotic Regimen - Native or Prosthetic Valve

Ceftriaxone 2 g IV q24h

OR

Ampicillin 2 g IV q4h

OR

Moxifloxacin 400 mg PO/IV q24h

Ciprofloxacin may be substituted for Moxifloxacin.
