  • Nasal colonization with MRSA

  • Injection drug use

Empiric IV Antimicrobial Regimen

Cefotaxime 50mg/kg IV Q8H
Max 6g/day

AND

Cloxacillin 50mg/kg/day IV Q6H
Max 6g/day

MRSA Risk Factors Present

Vancomycin 15-20 mg/kg IV Q6H
