MSSA Treatment / Infective Endocarditis

Native Valve Treatment

Nafcillin 2 g IV every 4 hr x 6 wks

OR

Cefazolin 2 g IV every 8 hr* x 6 wk

*For use in patients with nonsevere PCN allergy

Prosthetic Valve Treatment

Nafcillin 2 g IV every 4 hr x ≥6 weeks

PLUS

Rifampim 300 mg IV/PO every 8 hr x ≥6 wks

PLUS

Gentamicin 1 mg/kg IV every 8 hr x 2 weeks
