Antimicrobials
Based on local epidemiology, Ciprofloxacin should NOT be used for empiric treatment of UTI. If patient transferred to acute care, refer to UTI (Acute) algorithm.
Infection of either upper or lower urinary tract with functional or structural abnormality (examples: catheter, stent, strictures, stones, prostatic hypertrophy)
Cotrimoxazole DS tablet PO BID x 7 days
Does not cover Enterococcus spp.
OR
Amoxicillin-Clavulanate 875-125mg PO BID x 7 days
If symptoms do not resolve promptly (within 5 days) consider prolonging therapy to 10-14 days
Catheter should be removed or replaced
