Mild Infection

Presentation

Systemically ill or multiple comorbidities

Consult

Consider Infectious Disease Consult if failure to improve after 3 days

Initial Empiric Treatment

Cefazolin 2g IV daily

AND

Probenecid 1g PO daily given 30 min prior to IV Cefazolin

Do not use if CrCl < 30mL/min

Inpatients

Cefazolin 2g IV q8h

Penicillin Allergy

Penicillin allergy assessment

Clindamycin 300-450 mg PO TID

OR

Moxifloxacin 400mg PO daily

Follow Up

Reassess daily for PO stepdown. A positive response includes: resolution of fever, no rapid progression of affected area, improved local pain
