14 day duration of treatment is recommended
Most of the treatment should be completed with IV therapy with the exception of linezolid for MRSA, which can be used orally due to excellent bioavailability
Blood cultures are recommended to ensure absence of bacteremia
Infectious Diseases consultation is recommended
High vancomycin troughs (15-20mg/L) are recommended for MRSA pneumonia
Oral step down of beta-lactams may be considered in patients who are absolutely clinically stable. Use Cephalexin 500mg PO QID for MSSA
