First Recurrent Episode

Consult

Pharmacist Consult to pre-arrange outpatient medication coverage

Antibiotics

Vancomycin PO or NG Taper Regimen

  • 125 mg QID x 10 days then

  • 125 mg BID x 7 days then

  • 125 mg OD x 7 days then

  • 125 mg q2d x 7 days then

  • 125 mg q3d x 7 days then stop

OR

Fidaxomicin 200 mg PO BID x 10 days

Clinical Information

May use diluted IV vancomycin PO or NG if needed as it is difficult to remove powder from capsules (reconstitute 500 mg vial as per directions and then further dilute in 30 mLs of water prior to administration)
