Dosing

Rectal Dosing500mg in 100mL Normal Saline q6h x 10 days (may use capsules or IV solution)

Severe/Complicated C. Diff Infection Dosing500mg PO/NG q6h x 10 days (along with metronidazole IV)

Taper Dosing125mg PO/NG QID x 7 days then
125mg PO/NG TID x 7 days then
125mg PO/NG BID x 7 days then
125mg PO/NG OD x 7days then
125mg PO/NG q2d x 7 days then
125mg PO/NG q3d x 7 days then stop

Usual Dosing125mg po/ng q6h for 10-14 days

General Information

Common Usage

Only indication is in the therapy of C. difficile infection, particularly recurrent or severe disease.

Drug Monitoring

Possibility of accumulation in severe renal dysfunction.

Consider checking a vancomycin level in dialysis patients.

Additional Information

No oral absorption.

May increase vancomycin resistant Enterococci colonization in the gut.
