CeFAZolin IV 30 mg/kg/dose (Max 2000 mg/dose)*
*The maximum adult IV dose for CeFAZolin in patients ≥ 120 kg is 3 g/dose.

Tobramycin 2.5 mg/kg/dose (Max 120mg/dose)
If peritoneal or surgical field contaminated with bowel contents add:
Metronidazole 10 mg/kg/dose (Max 500mg/dose)
Every 12 hours
If MRSA positive, vancomycin should be given in addition to cefazolin, tobramycin, and metronidazole.
Vancomycin IV 15 mg/kg/dose (Max 1000 mg/dose)
