Antimicrobials
Culture of pleural fluid and/or endotracheal secretions strongly recommended
During annual influenza epidemics, consider concurrent testing for influenza and, if clinically indicated, treatment with Oseltamivir
If Mycoplasma or Chlamydia strongly suspected, please send appropriate testing.
Droplet / Contact Precautions
If viral etiology ruled out and clinical symptoms indicate bacterial cause and patient improving then precautions may be discontinued after 24 hours of antimicrobial therapy
S. pneumoniae
S. pyogenes (GAS)
S. aureus - Although uncommon,
may be a pathogen especially in the setting of influenza or other viral respiratory infections
CefTRIAXone IV
PLUS
Cloxacillin IV*
*If pneumatoceles or MSSA suspected. Re-evaluate once cultures become available.
Vancomycin IV*
*If MRSA suspected. Re-evaluate once cultures become available.
14 - 28 days depending on complications including oral conversion when afebrile for 48 hours
