Criteria
SIRS + potential infection + NO evidence of life-threatening organ dysfunction (below)
Piperacillin-tazobactam 3.375 g IV q6h
OR, IF ESBL RISK FACTORS:
Meropenem 500 mg IV q6h
Two or more of (qSOFA): 1. Respiratory rate ≥ 22/min 2. Altered mentation 3. Systolic blood pressure ≤ 100 mmHg OR Lactate greater than 4 mmol/L
+/-
Vancomycin
ALTERNATIVE:
Gentamicin 7 mg/kg IV q24h
