S. aureus

S. pyogenes (Group A strep)

S. pneumoniae

Empiric Antibiotic Options

Cefazolin 25 to 33 mg/kg IV Q8H

OR

Vancomycin 20 mg/kg IV Q6H

Penicillin Allergy*

Cefazolin 25 to 33 mg/kg IV Q8H

OR

Vancomycin 20 mg/kg IV Q6H

*Avoid all beta-lactams antibiotics (ie cephalosporins, carbapenems) if anaphylaxis to penicillin
