Most likely pathogens
Ceftriaxone 1 gram IV q24h X 10-14 days (including oral conversion)
OR
Ampicillin 1-2 grams IV q6h X 10-14 days (including oral conversion)
AND
Gentamicin 4-7 mg/kg/dose IV q24h x 10-14 days (including oral conversion) Maximum: 500 mg/24h prior to levels.
Ceftriaxone 1 gram IV q24h x 10-14 days (including oral conversion)
Note: If no clinical improvement or if status worsens after 72 hours of therapy, investigations for complications of renal infection or urinary obstruction should be undertaken.
