Cefazolin 16-33 mg/kg/dose IV q8h
Maximum: 2000 mg/dose

OR

Cloxacillin 37.5-50 mg/kg/dose IV q6h
Maximum: 12 grams/24h

Penicillin allergy

Cefazolin 16-33 mg/kg/dose IV q8h
