Cellulitis: Acute outpatient

Most likely pathogens

Streptococcus pyogenes (Group A Strep)

Staphylococcus aureus

Duration of therapy

5-10 days

Suggested regimen

Cephalexin 12.5 mg/kg/dose PO QID
Maximum: 1000 mg/dose

Penicillin allergy

Clindamycin 20-30 mg/kg/24h PO divided TID-QID
Maximum: 1.8 grams/24h

If MRSA suspected

Co-trimoxazole 4-6 mg/kg/dose PO BID
(dosing based on trimethoprim component)
NOTE: Co-trimoxazole does not cover Strep pyogenes
