Most likely pathogens
Give intravenous antibiotics until afebrile X 24 hours (minimum 36 hours of IV antibiotics with negative initial blood cultures)
Therapy can be completed with oral antibiotics after identification and sensitivities are available. Cephalexin or cefixime are the preferred oral agents, if the organism is susceptible.
Consider LP, if clinically warranted
10-14 days
Ampicillin 50 mg/kg/dose IV q6h
AND
Ceftriaxone 50-75 mg/kg/dose IV q24h
OR
Gentamicin 7 mg/kg/dose IV q24h
