Switch to oral therapy (amoxicillin-clavulanate) when patient afebrile and eyelid and orbital findings begin resolve
Uncomplicated: 14-21 days
Complicated: at least 28 days
Ceftriaxone 100 mg/kg/dose IV x 1 dose, then 12 hours later: Ceftriaxone 50 mg/kg/dose IV q12h Maximum: 2 grams/dose AND/OR 4 grams/24h
PLUS
Vancomycin 60 mg/kg/24h IV divided q6-8h Maximum: 4 grams/24h prior to levels
If intracranial extension see Cerebral Abscess for empiric therapy.
