3-4 weeks based upon organism and clinical response (4-6 weeks for hip)
Consider switch to oral antibiotics to complete therapy when clinically improved, inflammatory markers have started to normalize. C-reactive protein testing is recommended to monitor response to therapy and should be normal before stopping therapy.
Cefazolin 33 mg/kg/dose IV q8h Maximum:2000 mg/dose
OR
Only if MSSA isolated, as cloxacillin may cause phlebitis Cloxacillin 50 mg/kg/dose IV q6h Maximum: 12 grams/24h
If Cultures Positive for Group A Strep: Penicillin G 0.1 million units/kg/dose IV q6h Maximum: 24 million units/24h
