Tubo ovarian abscess

Duration of Therapy

Total course 10-14 days, if image guided drainage

If no drainage, minimum 14 days; continue until resolution on FU imaging up to 4-6 weeks

Completion of therapy with oral if clear clinical improvement with either:
(Amoxicillin-Clavulanate 500 mg PO q8h) OR (Levofloxacin 500-750 mg PO q24h AND Metronidazole 500 mg PO q12h)

Suggested Regimen

Cefoxitin 1-2 grams IV q6-8h

OR

Ceftriaxone 1 g IV q24h

AND

Metronidazole 500 mg PO/IV q12h

+/-

If positive for Chlamydia:

Doxycycline 100 mg PO q12h x 14d

Penicillin Allergy

Ceftriaxone 1 g IV q24h

AND

Metronidazole 500 mg PO/IV q12h

+/-

If positive for Chlamydia:

Doxycycline 100 mg PO q12h x 14d

OR

Levofloxacin 500-750 PO/IV q24h daily (use IV only if unable take PO)

AND

Metronidazole 500 mg PO/IV q12h (use IV only if unable take PO)
