Suggested Regimen

Cephalexin 500 mg PO q6h x 7 days

OR

Nitrofurantoin 50-100 mg PO q6h x 5 days
OR
Macrobid (Nitrofurantoin Monohydrate Macrocrystals) 100 mg PO q12h x 5 days
(Note: Nitrofurantoin should not be used in late 3rd trimester)

OR

Amoxicillin 500 mg PO q8h x 7 days
(Note: Should not be used empirically as high resistance rates with E.Coli and no activity against Klebsiella)

Penicillin Allergy

Co-trimoxazole 800/160 mg (1 DS tablet) PO q12h x 3 days
(Note: Co-trimoxazole should not be used in the 1st trimester OR 3rd trimester)

OR

Nitrofurantoin 50-100 mg PO q6h x 5 days
OR
Macrobid (Nitrofurantoin Monohydrate Macrocrystals) 100 mg PO q12h x 5 days
(Note: Nitrofurantoin should not be used in late 3rd trimester)
