10-14 days (including oral conversion, once afebrile for 48 hours based on culture and sensitivity)
Nitrofurantoin should NOT be used
Ampicillin 1-2g IV q6h
AND
Gentamicin 5 mg/kg/24h IV once daily. Maximum 500 mg/24h prior to levels
OR
Ceftriaxone 1 g IV q24h
