Tobramycin Peds Dosing

EXTENDED INTERVAL INITIAL DOSING *Consider alternate antibiotic if renal dysfunction at baseline*

Dosage adjustments are made based on serum concentrations, clinical response and renal function (see "Drug Monitoring").

7 mg/kg/dose IV q24h
Maximum: 400 mg/dose prior to levels

10 mg/kg/dose IV q24h
Maximum: 600 mg/dose prior to levels
