240-300 mg/kg/24h IV divided q6-8h Maximum: 4000 mg/dose Usual dosing regimens for children weighing greater than 40 kg: 3 grams IV q6h OR 4 grams IV q8h
75-100 mg/kg/dose IV q6h Maximum: 4000 mg/dose
