Co-trimoxazole Peds Dosing

Shortage of Oral Suspension

There is a long term shortage of co-trimoxazole suspension in Canada

Doses may need to be rounded to portions or whole tablet strengths to allow patients to crush for administration.
OR
Alternate choice of antimicrobial may be considered if liquid formulation is required (when clinically appropriate)

General, Greater than 2 months

Dosing is based on the trimethoprim component:
4-6 mg/kg/dose IV/PO q12h

Urinary Tract Infection (UTI)

Pneumocystis Jiroveci Pneumonia (PJP) Infection, Greater than 2 months/ Cystic Fibrosis

Dosing is based on the trimethoprim component:
15-20 mg/kg/24h IV divided q6-8h

Pneumocystis Jiroveci Pneumonia (PJP) Prophylaxis (immunocompromised patients), Greater than 2 months

Dosing is based on the trimethoprim component:
2.5 mg/kg/dose PO BID on two days of the week.
Maximum: 160 mg/dose
